To serve you better, we developed a new Beneficiary Information Form to make it easier to keep us up-to-date with the contact information of your beneficiaries. That way, when the time comes, we’ll know exactly who should receive the benefits of your policy.
If it has been a while since you last looked at your beneficiary designations, visit the AAFMAA member center now at connect.aafmaa.com and review your policy.
If your beneficiary’s contact information has changed, simply fill out a Beneficiary Information Form, available by selecting the “AAFMAA Forms” link under “My Documents.” If you need to change any of your designated beneficiaries, complete the Beneficiary Designation Form, which includes a beneficiary information request on the last page.
