Where should I send claim documents related to a death?
Our Survivor Assistance Services team can file claims on behalf of Armed Forces Mutual Members. Please visit the Member Center where you can upload the essential documents we’ll need to assist you. You can download our Military Survivor Pre-Planning Checklist there, too.

What is the typical timeframe of the claims process?
Do I need an original death certificate to make a claim?
What forms of government-issued ID are acceptable to claim benefits?
Which documents should be stored in the Digital Vault to help make the claims process faster and easier?
Please ensure we have copies of the following documents in your Digital Vault (if applicable to you):
- DD form 214
- Marriage certificate
- Divorce decrees for you or your spouse
- Death Certificate(s) for any former spouses and dependents
- VA award letter or VA rating decision (the official letter from VA that identifies the medical conditions that have been awarded service connection by the VA)
- Trust pages (if the beneficiary is on the life insurance policy)
- Recent Military Retired Pay Statement
- Birth Certificates
- Legal Documents (Wills, Powers of Attorney, Estate plans)
- Other Life Insurance Policies
- Civil Service or other private pensions
- Notification of disabled adult children
