What Your Survivor Benefits Representative May Request 

In order to swiftly process claims and secure survivor benefits for you, we suggest you visit the Member Center to ensure we have copies of the following essential documents, if they apply to you:

  • DD form 214
  • Marriage certificate
  • Divorce decrees for you or your spouse
  • Death Certificate(s) for any former spouse(s)
  • VA award letter or VA rating decision (the official letter from VA that identifies the medical conditions that have been awarded service connection by the VA)
  • Trust pages (if a trust is the beneficiary on the life insurance policy)
  • Recent Military Retired Pay Statement
